I/We_____________________________________,do hereby enter into a contract with Country Creek
Labrador for the purchase of a Labrador Retriever puppy. I agree to the following terms of this contract.
1. The puppy in reference is guaranteed to be in the best health to the breeders acknowledgement.
A 24 Month health guarantee will be given as followed. No joint Stressed activities until the puppies age of 24 months
old. Example: Over working their joints, having them jump out of pick ups, off high places!
2. All responsibilities of the puppy in reference are released upon delivery! The breeder
WILL NOT be responsible for ANY vet related fees such as Routine health check,vaccines due after delivery, or worms(IF
PUP IS SHIPPED TO YOU) . To qualify for any refunds you must have your puppy examined by a LICENSED VET of the new owners
choosing within 48 hours of delivery. IF the puppy in reference, in the opinion of a vet, has a physical problem, that
makes he/she a poor health risk, we will provide you with a healthy puppy. You must have your vet provide a written statement
showing dates and how your puppy is considered a poor health risk. The puppy owner will then be allowed 48 hours to return
the puppy to the breeder at the customers expense along with all registration applications signed back over to the breeder!
3. I agree to carry on the breeders name (COUNTRY CREEK) in the prefix or suffix of the
puppies registered name on all applications
Example: COUNTRY CREEKS BIG BOY
Example: BIG BOY OF COUNTRY CREEK
*This allows the breeder to carry thier name, Bllodline and keep track of their puppies!
4. This contract contains ANY information that I may need for my puppy. I have been provided
with full health records showing what my puppy has been treated for and when.
5. I agree that the puppy in reference comes with FULL registration privleges as long as I agree
to breed he/she responsibly adn under AKC guidelines!
The puppy in reference has been on a high performance brand dog food. The puppy will need to
be gradually weined off of this formula if you intend to change brands! The pup has been fed Black Gold/Green/Lamb &
Rice.
I do agree to all the terms and conditions of the contract and willfully enter
into contract with Country Creek breeder, Shanna Maynard on this the ___day of ______________,2006. I have been
briefed verbally on the characteristics and health regarding the puppy in reference. I hereby enter into this contract with
a deposit of 200.00, with the remainig balance due before airline departure or adoption date

***PLEASE carefully consider your commitment in proceeding with your adoption
as all deposits are NON-REFUNDABLE. The exception for a refund is if your puppy is not born in the reserved litter expected.
At that time you will be able to recieve a refund for the total amount paid or have your choice of picking another puppy!
